    Provides health promotion and population/panel based care delivery program leadership. Manages and facilitates new and existing initiatives to enhance care of designated populations endorsed by Region. With HPMG partner, supports regional CMI and links vision, expertise and direction for clinical practice improvement. Serves as role model. Supervises assigned staff.

    Essential Responsibilities:

  • Collaborates with population/panel management operational leaders on program operations including implementation, reporting, and program evaluation. Develops, designs, and redesigns care delivery programs to improve quality, service and resource management. Identifies key networking relationships. Works to actively integrate care management programs/initiatives with other aspects of KP delivery system. Works with key individuals from other population management programs to fully integrate delivery of care.

  • Serves as regional resource and program liaison between KP Hawaii CMI programs and National CMI. Communicates purpose and initiatives of National CMI in Hawaii Region. Shares interregional population management information including clinical practice ideas as appropriate.

  • Consults with, collaborates and responds to requests from local/national sales, marketing and operations staff to present and interpret KP population management strategies and programs. Demonstrates examples of value added guidelines.

  • Works in collaboration with national and regional leadership and 21st Century Care Innovation teams. Serves as project manager for project. Understands needs of KP-Hawaii population. Designs work and Builds care teams to meet needs. Develops relationship-based care program. Provides alternatives to receiving care instead of office visits (e.g., telephone, group, email, letters). Embraces Total Panel Ownership. Engages members and family in Collaborative Care Planning to actively engage in decisions about member's care and support to manage health.

  • Works to improve metrics/outcome measures and CMI Memorandum of Understanding (MOU) to include both national (core) and local goals. Represents MOU information at regional meetings and on local committees.

  • Provides periodic status reports on state of CMI and Disease Management to local leadership.

  • Represents CMI and disease management on regional and national committees.

  • Develops and presents business cases to support need for additional monetary resources and/or FTE's to expand or improve local program.

  • Participates in development of programs that prompt physicians at point of care and managers can use to track individual/team/clinic/regional performance.

  • Assists with Panel Support Tool continued deployment in reach/outreach workflows to close care gaps.

  • Maintains clinic practice and skills to assist in applying theory into practice.

  • Supports innovation, new technologies, and new care delivery models.

  • May perform patient care to the extent necessary to maintain clinical expertise, competency and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.

  • Provides direct patient care on an as needed basis. Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.

  • Incorporates the KP Nursing Vision, Model and Values through out their Nursing Practice.

  • Hires, trains, supervises, counsels, disciplines, and terminates assigned staff as appropriate.

  • Communicates goals, objectives, accountabilities, priorities, and authority parameters to assigned staff.

    Basic Qualifications:

    Experience

  • Minimum five (5) years clinical health care experience including program development and implementation and/or project management.

  • Minimum two (2) years of management experience.

  • Education

  • Bachelor degree in Nursing.

  • License, Certification, Registration

  • Valid Hawaii RN license (must meet education requirement(s) for Hawaii State licensure) upon date of hire.

  • Basic life support for the health care professional or CPR/AED for the Professional Rescuer certification.

  • Valid driver's license.

  • Additional Requirements:

  • Excellent patient assessment skills.

  • Demonstrated ability to coordinate care and sensitivity to chronically ill populations and their caregivers/families.

  • Demonstrated ability to plan, analyze and explain data and its implications.

  • Demonstrated knowledge of and skill in change management, innovation, customer service, group process facilitation, conflict resolution, influence, interpersonal relations, oral communication, problem solving, project management, quality management, systems thinking, team building, teamwork, and written communication.

  • Demonstrated knowledge of and skill in word processing, multimedia presentations, and spreadsheet PC applications.

  • Preferred Qualifications:

  • Electronic medical record experience.

  • Knowledge of customer quality issues, service delivery planning and quality improvement methods, organizational development principles and adult learning.

  • Knowledge of disease management practices and processes.

  • Certification in area of specialty.
